Community care services provide opportunities for Medicaid beneficiaries, low-income individuals, veterans and families that need support to care for a loved one. Beneficiaries can receive services in their own home or community without being required to move to a facility or care home.* These programs serve a variety of targeted populations, including the elderly or those with disabilities.
Community care provides services that are primarily non-medical, including homemaker duties, respite and assistance with personal care needs.
Some programs may also offer nursing, therapies, home modifications and case management services. You can maintain your independence within the comfort of your home. Our home care professionals come to you.
We also coordinate your care with other providers to ensure that you receive most complete, appropriate care possible to maintain your health and wellness.
Authorized services may include:
- Assist with personal care, such as bathing, grooming and dressing
- Light housekeeping and laundry
- Meal preparation

- Accompany client to appointments
- Respite care for family caregivers
- Case management
- Minor home modifications
